(79) Patient Characteristics Prior to Transition to Adult Care at a Single Pediatric Kidney Transplant Program (2016-2021)

Background: Transition from pediatric to adult care is a high-risk period for graft loss in young adults with kidney transplants. Though social determinants of health are known to influence outcomes, institution-specific risk factors remain poorly characterized.

Objectives: Describe clinical and social characteristics of patient cohort at time of transition from pediatric to adult care.

Design/Methods: Retrospective chart review of patients seen by pediatric transplant nephrology at the University of Michigan who transitioned to adult care between 2016–2021. Data included demographics, clinical outcomes, lab monitoring, appointment attendance, and characterization of post-transition nephrology care. eGFR calculations were made utilizing the combined creatinine-cystatin C CKiD U25 equation.

Results: Image 1 describes the cohort (N=41). Image 2 shows monitoring and adherence parameters at time of transition, including missed/no-show appointments (Panel A), missed monthly labs (Panel B), and tacrolimus levels out of range (Panel C). Image 3 highlights the prevalence of four common mental health diagnoses in the cohort.

Conclusions: This descriptive analysis underscores the heterogeneous clinical and social circumstances facing individuals with kidney transplant at time of transition from pediatric to adult care. Further analysis of these data will aim to more precisely identify modifiable risk factors associated with graft loss to inform targeted interventions and improve outcomes for this at-risk population.
